Are you in need of our services? 

We have limited sizes and articles at this time, so there is a possibility that we may not fulfill all inquires at this time.

Requirements:

• Print the Referral Form Click Here (right button click, select print)
• Print the Agreement Form Click here  (right button click, select print)
• Print the Photo Release Form Click Here  (right button click, select print)
• Complete the form along with a parent/guardian and school staff member (we never ask for proof of income or financial records) 
• Scan/Email,  

An email will be returned to you within 5 days of receiving and an appointment will be set up for you. I'm trying to work with the schools, so that we can come to you and meet privately.  If not a public location in Manitowoc County will be the location of the appointment.
